This item looks like a clear glass bottle with a cork, but swirling black clouds and brief flashes of electricity can be seen inside it. When the cork is removed as a standard action, the storm inside spills out. This has different effects depending on where the cork is when it is removed.
If you are outdoors when you remove the cork, a thunderstorm is called forth in a 2-mile radius area from the bottle. It takes one minute for the thunderstorm to manifest, and the thunderstorm lasts for 3d12 hours.
If you are indoors or underground when you remove the cork, a 60-foot radius storm cloud is called forth. It takes one round for the cloud to form. The cloud obscures all sight, including darkvision, beyond 5 feet. A creature within 5 feet has concealment. Creatures farther away have total concealment. Any creature in the cloud has a 75% chance of being struck by a bolt of electricity each round. Anyone struck by a bolt takes 5d6 points of electricity damage and is dazzled for 1 round (DC 17 Reflex save halves the damage and negates the dazzle effect).
If you are underwater when you remove the cork, the bottle immediately deals 8d10 electricity damage to all targets in the water within 120 feet, including you. A DC 17 Reflex save halves this damage.
A bottled tempest can only be used once before the tempest inside is lost forever and the bottle becomes nonmagical. Unless the cork is pulled underwater, reinserting the cork before one round has passed cancels the effect, allowing you to use the item at a different time.
